I did a Google image search for "invoice" and got a bunch of results. ( http://images.google.com/images?sourceid=navclient&ie=UTF-8&rls=RNWE,RNWE:2004-43,RNWE:en&q=invoice )
Basically you want your contact information, his contact information, the dates, a brief description of the job, and the amount of money exchanged.
